Under its December 2025 three-year most-favored-nation (MFN) agreement with the U.S. government, Merck provides Januvia, Janumet and Janumet XR through a direct-to-patient program and plans to expand it to Lipfendra. Merck also agreed to discounted Medicaid pricing, joined CMS’s GENEROUS Model in June 2026, applied MFN pricing to most products launched during the agreement, and agreed to share a portion of foreign revenue resulting from successful U.S. trade-policy efforts. Separately, Merck reached an agreement with the U.S. Department of Commerce to delay Section 232 tariffs for three years while it invests in reshoring manufacturing for U.S. patients.
