Commercial insurers are expanding discounted payment mechanisms, including predetermined payment and DRG-based systems, across more inpatient and outpatient services. UHS stated that lower private-insurance reimbursement could negatively affect hospital operating results if payer reductions exceed corresponding cost reductions. The No Surprises Act federal independent dispute resolution process has experienced significant claim-processing delays because of provider litigation, court-vacated regulations and repeated CMS pauses and resumptions. A June 4, 2026 final rule is intended to improve the federal IDR process, but UHS cannot yet predict its operational impact.
